Is the hook threaded to the eye bolt?  Can the hook be detached?

Crosby uses a threaded round nut with a special "skirt" to cover the thrust bearing.  Drive out the roll pin and unthread the round nut to get to the thrust bearing.

What size shackle goes with this hook?

There is not a single right answer, but perhaps the most common choice would be the Crosby 1" G-209, which is rated @ 8.5T.  Unless there is a very specific reason not to, you want your shackle rated equal to or greater than the hook.
